Event Info
The Youth Group of Emmanuel Baptist Church will be attending the Corn Maze in Exeter, MO. They will need to arrive at the church on Wednesday, September 24th by 4:30pm and will return around 9:00pm.
Permission
I give permission for my student (named above) to attend the Corn Maze in Exeter, MO. I further give permission for my student to be transported to and from the event by approved adult volunteer drivers authorized by Emmanuel Baptist Church.
Photo Release
I recognize that EBC uses photographs and video images of events in church publications including websites and newsletters. I hereby grant permission for photo/video images of my student to be taken and used for such purposes.
Activity Release
I further give permission for my student to participate in all supervised activities except as noted:
